Did anyone else get an email from a Hector Contreras, Del Rey mktg Agency saying i have to open and complete a Prize Release form as apotential winner of $100 publix gift card??
In the past the winning gift card just showed up in the mail. I’m afraid to open form in case it isa scam/virus.

I also received the same email. I am concerned that it is a scam as you should only have to provide an affadavit of eligiblity if ithe prize is over $600

I got confirmation email from the marketing agency that they received my release form and were processing the gift card. I will update here if I get it in the mail.

I received my $100 Gift card, and spent it already!
First I got a phone call 5/26 from Del Rey Marketing and then received the email with the form on 5/28. It seems many people did not respond to however they notified winners initially (maybe out of scam suspicions) so new winners (including me!) were picked later.
